I recently got sent a questionaire about speech problems etc .
To be honest i should have copied it before sending it back ,BUT my brain wasn't in gear at the time 
Some of the questions asked are below , and for me it did make me think
have you / did you have problems pronouncing certain words/getting words out (ie did you / do you get tongue tied ?)
Is swallowing a problem / or has it been in the past ?
Does your mouth fill with Saliva
Do you / have you had repetitive ear infections / perferation of the ear drum
Do you have problems with speaking sometimes , ie does it feel like you have got no power vocally / and also you can not shout .
for me
have you / did you have problems pronouncing certain words / getting words out (ie did you / do you get tongue tied ?) =yep a lot as a kid / still have problems now
Is swallowing a problem / or has it been in the past ? =definatley all my life
Does your mouth fill with Saliva = YEP which is a pain at the dentist
Do you / have you had repetitive ear infections / perferation of the ear drum = i have had ear infections , no constant , maybe 1 every year or two
Do you have problems with speaking sometimes , ie does it feel like you have got no power vocally / and also you can not shout .= yes have this too
